Join Pennie's Home Health as a Registered Nurse (RN) in Fort Lauderdale, Florida! We are seeking a compassionate and experienced Registered Nurse to provide exceptional home health care services. This full-time position requires a current RN license and at least one year of nursing experience. Qualifications / Educational Requirements: Must be a Registered Nurse (R.N.) with a valid, current license. Minimum one year of nursing experience as an RN. Ability to work positively and collaboratively with patients, families, and healthcare staff. Responsibilities / Essential Functions: Conduct initial home care patient visits and regularly re-evaluate patient needs and progress. Initiate and implement the plan of care under physician orders. Complete admission, transfer, re-certification, resumption of care, and discharge OASIS documentation. Observe, assess, and document patient symptoms and monitor progress. Educate patients and caregivers on disease processes, medications, care plans, and home health techniques. Coordinate patient services and supervise Licensed Vocational Nurses (LVNs) and Home Health Aides (HHAs). Notify physicians and clinical staff of any changes in patient condition promptly. Perform skills according to agency policies and procedures. Discharge patients from skilled nursing services when appropriate. Participate in case conferences to ensure coordinated care. Maintain accurate clinical records in compliance with agency standards. Attend ongoing education and training to maintain and update nursing skills. Adhere strictly to Medicaid and Medicare regulations. Work Environment and Physical Requirements: This role involves travel to patient homes within Fort Lauderdale and requires physical ability to perform nursing duties in a home care setting.
