Key Responsibilities Deliver compassionate patient-centered care by performing thorough assessments, monitoring vital signs, and accurately documenting patient progress to ensure high-quality outcomes. Administer medications and treatments as prescribed, maintaining strict adherence to safety protocols and regulatory standards. Educate patients and their families on care plans, medication regimens, and effective health management strategies to promote patient autonomy and well-being. Maintain precise and timely documentation of all patient care activities and interactions using electronic health record (EHR) systems to support continuity of care. Collaborate closely with a multidisciplinary healthcare team, including Registered Nurses (RNs), to develop and implement individualized care plans tailored to each patient’s unique needs. Participate actively in regular team meetings and case discussions aimed at continuously improving patient care methodologies and outcomes. Build strong, empathetic relationships with patients and their families to foster trust, comfort, and effective communication throughout the care process. What We're Looking For Required: Valid Licensed Vocational Nurse (LVN) certification in the state of Texas, ensuring compliance with state nursing regulations. 1-3 years of clinical or private duty nursing experience, demonstrating proficiency in patient care and medication administration. Strong skills in vital signs monitoring, patient assessments, and documentation accuracy. Excellent communication skills with keen attention to detail and safety awareness. Preferred: Experience working with electronic health record (EHR) systems to enhance care documentation and coordination.
