Position Overview We are seeking a compassionate and skilled Master Social Worker (MSW) fluent in Spanish and English to provide Comprehensive In-Home Care (CIC) and Independent Living Skills Training (ILST). The MSW will collaborate directly with individuals to create and implement person-centered plans that support their goals, preferences, and promote autonomy within their communities. Key Responsibilities Conduct initial and ongoing assessments to evaluate individuals’ needs, strengths, preferences, and challenges. Develop and implement personalized care plans that encourage independence and community integration. Provide ongoing support and coaching in home or community settings as outlined in care plans. Assist with daily living activities including meal preparation, medication management, and personal hygiene. Train individuals and families on budgeting, household management, and effective communication skills. Coordinate care with healthcare professionals, therapists, and case managers to ensure comprehensive support. Qualifications Master’s Degree in Social Work (MSW) from an accredited program. Fluency in both Spanish and English required. Experience working with individuals in home healthcare or community-based settings preferred. Strong interpersonal and communication skills.
