Job description

About the role

Impresiv Health is recruiting NPs and PAs across Arizona and Florida to provide personalized, patient-centered care to individuals living with Chronic Kidney Disease (CKD) and other complex chronic conditions.

Tasks

Combination of in-home visits, telehealth, and care coordination

Requirements

Licensed and board-certified Nurse Practitioners or Physician Assistants
APP experience in nephrology, cardiology, critical care, internal medicine, hospital medicine, endocrinology, or complex primary care preferred
Prior RN experience in cardiac ICU, medical ICU, emergency medicine, Med/Surg, cardiology, or nephrology is a plus
Experience managing patients with complex chronic conditions preferred
Value-Based Care (VBC) experience is a plus, but not required

Schedule

Monday–Friday, 8 AM–5 PM
No routine weekends or overnight shifts
Approximately 5–6 patient visits per day
