About the role
The Segue Residential Transition Program assists individuals transitioning out of state-run psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. Services help clients learn to manage their illnesses and develop independent living skills.
Responsibilities
- Oversee program development, coordinate care, and supervise staff, focusing on the day-to-day operations of the in-reach and service coordinator teams.
- Oversee in-reach processes and ensure services are authorized and compliant with guidelines.
- Supervise staff and support effective onboarding, training, and development.
- Assist with developing and managing the program budget, policies, procedures, and quality improvement systems.
- Maintain compliance with licensing, accrediting, and funding standards, and implement corrective action plans as needed.
- Conduct comprehensive clinical assessments, develop treatment plans, and implement interventions for individuals with serious mental illness, including co-occurring disorders.
- Support clients involved in the forensic system, including compliance with conditional releases and probation requirements, and collaboration with mental health courts.
- Develop and implement strategies to support transitions from long-term hospital stays to community living.
