Clinical Pharmacist

We are seeking a dedicated Clinical Pharmacist to join our team in Dover, DE. Key Responsibilities Conduct comprehensive medication reviews and provide patient counselling on proper medication use. Collaborate with healthcare professionals to develop individualized medication therapy plans. Monitor patient outcomes and adjust medication regimens as necessary. Educate patients and healthcare staff on medication therapies, side effects, and interactions. Maintain accurate and complete patient medication records and documentation. Participate in interdisciplinary team meetings to contribute medication expertise to treatment plans. Stay current with relevant pharmaceutical developments and best practices. Required and Preferred Qualifications Bachelor's Degree in Pharmacy required; PharmD preferred. Strong clinical skills with a focus on patient-centered care. Experience in clinical pharmacy settings, ideally 2-5 years. Current state licensure as a pharmacist. Familiarity with electronic health record systems and pharmacy management software.
