Job description
Systemart LLC is hiring a full-time Clinical Pharmacist in Dover, Delaware. In this on-site role, you will support patient-centered care through medication management, collaborate with healthcare professionals, and help improve patient outcomes.
Key Responsibilities
- Conduct comprehensive medication reviews and counsel patients on proper medication use.
- Collaborate with healthcare professionals to develop individualized medication therapy plans.
- Monitor patient outcomes and adjust medication regimens as appropriate.
- Educate patients and healthcare staff about medication therapies, side effects, and interactions.
- Maintain accurate patient medication records and documentation.
- Participate in interdisciplinary meetings and contribute medication expertise to treatment plans.
- Stay informed about pharmaceutical developments and current best practices.
Required and Preferred Qualifications
- Bachelor’s degree in Pharmacy required; PharmD preferred.
- Current pharmacist licensure in the state of Delaware.
- Strong clinical skills and commitment to patient-centered care.
- Two to five years of clinical pharmacy experience preferred.
- Familiarity with electronic health records and pharmacy management software.
- Board certification in a pharmacy specialty is a plus.
Team and Compensation
Report to the Pharmacy Manager and work closely with physicians, nurses, and other healthcare providers in a collaborative care environment. The listed annual pay is $200,000 per year.
