Registered Nurse (RN) – Accountable Care Organization (ACO)

Location: Central Maine Medical Center, Lewiston, ME

Full-time | Days | 8 -Hour Shifts

The Registered Nurse (RN) provides safe, patient-centered care within the Medical Group / Accountable Care Organization (ACO) setting. Delivers direct patient care, supports care coordination across the continuum, and ensures efficient patient flow and population health outcomes in accordance with clinical standards and scope of practice.

Central Maine Healthcare (CMH), a member of Prime Healthcare Foundation, is an integrated healthcare delivery system serving approximately 400,000 residents across central, western, and midcoast Maine through a network of more than 40 primary and specialty care locations. The system includes Central Maine Medical Center (CMMC) in Lewiston, a 250-bed, not-for-profit regional referral hospital that is home to the region's only Cancer Care Center, an orthopedic ambulatory surgery center, and the Central Maine Heart and Vascular Institute. CMMC also serves as the central Maine base for LifeFlight of Maine, the state's only medical helicopter service.

Responsibilities

Provide direct patient care in an outpatient/clinic settingCoordinate care plans with physicians and interdisciplinary teamsMonitor and communicate changes in patient condition, diagnostics, and symptomatologyRespond quickly and accurately to changes in condition or treatment responseSupport chronic disease management and population health initiativesGuide and support clinical staff while maintaining standards of careCollaborate across departments to ensure quality care, continuity, and patient flowEnsure adherence to regulations, policies, and clinical standardsParticipate in performance improvement, quality initiatives, and staff educationServe as a clinical resource to staff and team membersMaintain accurate documentation and ensure HIPAA compliance

Qualifications

Current state RN license. Current BLS certificate upon hire and maintain current. One year of professional nursing experience in a clinic setting, preferred.

Pay Transparency

Central Maine Healthcare Corporation offers competitive compensation and a reasonable compensation estimate for this role is $36.00 to $49.16 per hour. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
