Key Responsibilities Conduct comprehensive clinical assessments to accurately evaluate patients’ health status and individual needs. Develop , implement, and regularly update personalized care plans in collaboration with patients, families, and healthcare professionals. Administer medications and perform medical procedures including IV therapy and wound care with precision and adherence to safety protocols. Document patient progress meticulously, adjusting care plans promptly based on changes in health conditions. Coordinate care effectively by communicating with multidisciplinary teams to ensure seamless continuity of care. Educate patients and their families on health management, treatment options, and preventive measures to empower self-care. Ensure compliance with all regulatory standards and best practices in home healthcare delivery. What We're Looking For Required: Bachelor of Science in Nursing (BSN) degree. Valid Registered Nurse (RN) licensure in Texas. Clinical expertise in patient care, wound management, and IV therapy. Minimum 1-2 years of nursing experience in clinical or home health settings. Strong documentation skills and proficiency in care coordination. Excellent interpersonal and communication abilities. Preferred: Previous experience in Home Health nursing or similar community-based care. Familiarity with electronic health records (EHR) systems.
