Registered Nurse (RN) – Home Health Field Nurse in Houston, TX

kiwi consulting group is seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team in Houston, TX. As a full-time field nurse, you will provide personalized, patient-centered care directly in patients’ homes, making a meaningful impact on their health and well-being.

Key Responsibilities

• Conduct thorough clinical assessments to evaluate patients’ health status and care needs.
• Develop, implement, and regularly update individualized care plans in collaboration with patients, families, and healthcare professionals.
• Administer medications and perform medical procedures including IV therapy and wound care.
• Document patient progress accurately and adjust care plans based on changes in condition.
• Coordinate care with multidisciplinary teams to ensure seamless continuity of care.
• Educate patients and families on health management and treatment options.
• Maintain compliance with all regulatory standards and best practices in home healthcare delivery.

Required and Preferred Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Current Registered Nurse (RN) licensure in Texas.
• Clinical expertise in patient care, wound care, and IV therapy.
• Experience in Home Health or similar nursing fields preferred.
• Strong documentation and care coordination skills.
• Excellent interpersonal and communication abilities.
• Minimum 1-2 years nursing experience in a clinical environment.
