About the role

Adaptive Home Health is building an AI-native physical care platform, starting with home health. The branch Clinical Manager owns day-to-day clinical quality, staff development, and rehospitalization prevention. The role focuses on active clinical oversight, skills training and development, and systems and behaviors that help keep patients safe and out of the hospital.

Branch clinical leaders receive centralized support for back-office functions. Technology also reduces administrative work for field clinicians.

Responsibilities

• Own clinical execution and day-to-day clinical quality for the branch.
• Provide direct clinical guidance to clinicians as needed.
• Respond to clinical calls from patients, caregivers, and providers, and identify appropriate actions using clinical judgment.
• Monitor daily on-call events for urgent changes in patient status.
• Evaluate reports from assessing clinicians in real time to identify immediate patient needs and ensure the care plan.
• Develop staff and drive rehospitalization prevention.
• Partner with operations, scheduling, and leadership to keep care delivery on track and support safe scaling.
